questions that ask you for ratings
(1 to 5 scale) and narrative
responses to each question.
Please allow about one hour to
take each of the four surveys
beginning November 7, 2008 .
The survey will be closed on
November 12th at 10am (Texas
time) and results will be made
available at 2PM same day.
NOTICE: If you don't know the
answer to a question, then make
your best estimate of what you
think is correct or appropriate.
Each of the four surveys may take a
little over an hour and do not all
have to be done at one time.
Follow on screen instructions. Be

What Happens After the ACI-Xray is Completed?
1. First we go over the results with you in an online meeting.
2. Depending upon how the survey comes out, we may recommend that your
organization consider our HPO21 Program aimed at overcoming any weaknesses
found in the survey. You will learn more about this program after the survey.
